Intake Form
Head Start is available for low-income children and families, for foster children of any income and for children with diagnosed special needs of any income.  If you are interested in the Head Start program for your 3, 4 or 5 year old child, please fill out the intake form.  We will receive your form and give you a phone call.  Please call the Head Start office located in your county if you have any questions about the program:

Income Qualifications
Head Start enrollment is based on the income levels of 90% of participating families.  The U.S. Government establishes these guidelines annually, known as the “Federal Poverty Guidelines” as found in the chart below.  This is just a guide, there are many circumstances which may qualify your child for the Head Start program.  Please call the office in your county or fill out the intake form and you will receive a call back from a Head Start staff person.
